Sports Physicals
Sports physicals are good for three years. Please turn in the MSHSL Medical Eligibility Form to the Activities Office.
The system will not allow registration if the sports physical is more than three years old or if it is set to expire during the season you are registering for.

Check Your Student's Sports Physical Status
- In Affinety, select 'My Account' on the upper-left part of the screen.
- Log into your account and select the 'Medical' tab to determine whether the physical is current or not.
