Sports Physicals
Sports physicals are good for three years. Please turn the MSHSL Medical Eligibility Form into the Activities Office.
Affinety will not allow registration if the sports physical is more than three years old or if it is set to expire during the season for which you are registering.

Check Your Student's Sports Physical Status
- In Affinety, select My Account on the upper-left part of the screen.
- Log into your account and select the Medical tab to determine whether the physical is current or not.

Fusion (Formerly Galaxy) Registration
Targeted Services programming will be offered Mondays through Thursdays from 2:45 – 4:45 p.m. on days that school is in-person.
The first hour of instruction will be led by a licensed teacher. During the first hour, students will work on current concepts being taught during the school day, homework completion, test preparation, organization skills and study skills.
The second hour is focused on Social Emotional Learning (SEL), recreation and enrichment activities. During the second hour, students will be able to participate in programming such as sports, arts and crafts, technology and other activities based on student interest.
