At each school, nurses support Bloomington students by...
- Monitoring immunization status of students
- Writing and managing health plans for students with health conditions
- Providing consult and input for special education teams
- Providing first aid and emergency care to students and staff
- Administering medications
- Providing health care treatments and procedures
- Maintaining health service records
- Implementing screening programs
- Prevention and surveillance of communicable diseases
- Providing care coordination between school and health care provider
- Providing health resources and education to students and families
Special Medical Needs
The Building Nurse, working with the Licensed School Nurse, the Director of Health Services, the parent or guardian, and the student's Licensed Health Care Provider, develops a plan appropriate to the student's needs — this may be a Section 504 Plan, a condition-specific provider action plan (such as an Asthma Action Plan, Seizure Action Plan, or Diabetes Management Plan), or, for students receiving special education services, care plan content embedded in the student's IEP. Plans are reviewed annually. Please contact your school health office if your child has medical concerns that may require a school plan.

Medications

| Medication Administration Authorization | Required for any prescription or OTC medication given at school. Needs a health care provider order and parent/guardian signature. Covers up to 3 medications per form. |
| Student Self-Administration Authorization | Authorizes a student (grades 6–12) to carry and self-administer OTC pain relievers. |
| Topical Product Administration Authorization | For sunscreen (staff-assisted), insect repellent, and other OTC or medicated topical products requiring staff assistance. |

Emergency and Condition-Specific Care Plans

| Asthma Action Plan | For students with asthma; completed with your child's health care provider. |
| Seizure Action Plan | For students with a seizure disorder; provider- and parent-signed. |
| For students with diabetes; outlines monitoring, insulin, and emergency response. | |
| G-Tube Feeding Order | For students requiring gastrostomy tube feedings or medications at school. |
| CGM Monitoring Authorization | Authorizes read-only continuous glucose monitor (CGM) access by school health staff. Supplements, but does not replace, the Diabetes Management Plan. |
