Activities & Athletics Registration

Sports physicals are good for three years. Please turn the MSHSL Medical Eligibility Form in to the Activities Office.

Affinety will not allow registration if the sports physical is more than three years old or if it is set to expire during the season for which you are registering.

Check Your Student's Sports Physical Status

1. In Affinety, select My Account on the upper-left part of the screen.
2. Log into your account and select the Medical tab to determine whether the physical is current or not.
